Benzocaine hydrochloride overview:



Benzocaine hydrochloride is a hydrochloride form of the local anesthetic benzocaine, the pharmacological effect is the same as benzocaine, and the anesthetic effect is weaker than that of procaine. Local absorption is slow and the effect is long-lasting. Because it is insoluble in water, can not be used as infiltration anesthesia, has analgesic, antipruritic effect, drugs are usually made into powder or ointment, clinically used to treat wounds, ulcers, burns, skin abrasions and hemorrhoids.

 

Benzocaine is also known as ethyl paraaminobenzoate, cinesicin. The precipitated from water is colorless or white orthorhombic crystal system needle crystal or crystalline powder, and the precipitated from ether is square crystal. Odorless, bitter taste. The relative molecular weight is 165.19, melting point 92 °C, boiling point 310 °C, 183~184 °C (1.87x103Pa). Insoluble in water, soluble in ethanol, ether and chloroform, also soluble in almond oil, olive oil and dilute acid. Stable in air. With p-nitrotoluene as raw material, in sulfuric acid, sodium dichromate oxidation to generate p-nitrobenzoic acid, esterification reaction with ethanol, separation after the completion of the reaction, ethyl p-nitrobenzoate in acetic acid solution, with iron powder at 85~95 °C reduction, can obtain benzocaine crude product. After separation, it is washed with water and recrystallized in ethanol for refining.



Benzocaine hydrochloride dosage:


1. Containment: one tablet 6mg, 1 tablet at a time, 4 times a day.

2. 20% suspension for ear: adults can use 4~5 drops at a time, drip into the external auditory canal, and can be repeatedly administered for 1~2 hours as needed, and children do not use.

3. 5%, 20% ointment: adults for hemorrhoids, apply to the affected area, morning, evening and after stool, children do not use.

4. 20% gel: mainly used for the gum affected area in the mouth, and the gel for children is 5%.

10%-20% spray: spray on the affected area, repeat as needed, children should use with caution.


Benzocaine hydrochloride is suitable for:


1. Benzocaine hydrochloride topical agent is limited to the surface of the skin, should not be used in a large area, avoid contact with eyes and other mucous membranes (such as mouth, nose, etc.). If you are using other medicines, consult a doctor or pharmacist before using this product.

2. Patients with a history of allergy to benzocaine or other caine-type local anesthetics, such as procaine, lidocaine, etc., are banned.

3. Those with a history of methemoglobinemia are banned. Infants under 6 months of age are contraindicated.

4. Pregnant and lactating women should be used under the guidance of a physician.

5. Children under 5 and 2 years old should be used under the guidance of a physician, children over 2 years old should be used under adult supervision, and lozenges and gels should not be used for children under 2 years of age. The dosage for the elderly is the same as for adults.

6. Cholinesterase inhibitors can inhibit the metabolism of benzocaine hydrochloride, if you are taking such drugs, you should consult a doctor before using this product.


Benzocaine hydrochloride adverse reactions:


1. Less common adverse reactions: urticaria, angioedema, contact dermatitis, burning sensation and insect stinging sensation are less common;

2. Occasional adverse reactions: occasional dizziness, nausea, local or systemic allergic reactions, superficial desquamation of oral mucosa. Long-term use can make the oral mucosal surface and teeth colored, tongue blackening, taste changes, pharyngeal burning sensation, and can gradually recover after stopping the drug;

3. Serious adverse reactions: benzocaine can cause methemoglobinemia after use, resulting in shortness of breath, fatigue, confusion, headache, dizziness, nausea and heart rate changes and other symptoms, these symptoms can occur in a few minutes to 1-2 hours of the use of benzocaine, the first use and multiple use can occur, in rare cases methemoglobinemia can also lead to paralysis, coma and even death.
